How to Register Your Child

Stop in the Central Office Monday through Friday (except Holidays) 8 am until 3:30 pm to pick up the forms, or download them from the Registration Forms page and fill them out.

Items to bring to school or send scanned copy to us with the rest of the forms.

  1. Original Certified Birth Certificate

  2. Social Security Card

  3. Immunization Record

Please fill out the lunch form even though you will not be eating lunch in the building, it helps with grant funding.

If you are Native American fill out the “Title VII Student eligibility Certification”
