Stop in the Central Office Monday through Friday (except Holidays) 8 am until 3:30 pm to pick up the forms, or download them and fill them out.

Items to bring to school.

1. Original Certified Birth Certificate

2. Social Security Card

3. Immunization Record

4. Copy of Parent Identification
